Families often know something is wrong before anyone else. But raising concerns within complex care systems can be exhausting, confusing and sometimes met with defensiveness. ORVIA exists to provide an independent perspective — one that takes family concerns seriously and examines them with care, honesty and professional rigour.

Published safeguarding reviews and sector learning have consistently shown that families are often the first to sense that something is wrong — sometimes long before formal systems identify a concern. Yet research into safeguarding escalation has found that family voices are frequently dismissed, minimised or treated as complaints rather than intelligence. Where families are excluded from meaningful oversight, harm can continue because nobody is looking at the full picture.
