Safeguarding Audit
Independent, evidence-led safeguarding audit for health and social care providers. We examine what is actually happening — not just what paperwork says should be happening.
What Is a Safeguarding Audit?
A safeguarding audit is a structured, independent examination of how effectively an organisation protects the people in its care. It looks beyond policies and procedures to assess whether safeguarding is genuinely embedded in day-to-day culture, decision-making and practice.
Unlike a CQC inspection or internal compliance check, a safeguarding audit conducted by ORVIA focuses on operational reality — what staff actually do, what residents actually experience, and whether the systems designed to protect people are working as intended.
Who Is a Safeguarding Audit For?
- Care home operators seeking independent assurance that safeguarding practice matches policy
- Supported living providers preparing for regulatory scrutiny or responding to concerns
- Local authority commissioners who need confidence in provider safeguarding standards
- NHS Integrated Care Boards (ICBs) overseeing safeguarding across commissioned services
- Housing associations with supported housing or extra-care schemes
- Learning disability and autism services where the people you support may not be able to raise concerns themselves
ORVIA Lens™
An independent, intelligence-informed review of any care service.
For commissioners, oversight bodies, and safeguarding leads who need to know the truth.
Looking for a safeguarding consultancy? If you’re searching for safeguarding consultancy, you may be looking for independent oversight. ORVIA provides independent safeguarding audits and reflective reviews — not consultancy. Here’s how our approach is different.
When Should You Consider a Safeguarding Audit?
- Following a safeguarding incident, concern or near-miss
- When CQC has raised concerns about safeguarding practice
- As part of proactive governance and quality assurance
- When families or staff have raised safeguarding worries
- Before or after a Safeguarding Adults Review (SAR)
- When you suspect safeguarding drift — where standards have quietly slipped over time
- When commissioning new providers or reviewing existing contracts
What Our Safeguarding Audit Covers
Every ORVIA safeguarding audit follows our ORVIA framework — Observation, Reflection, Visibility, Insight, Accountability. We examine:
- Safeguarding culture — Is safeguarding genuinely understood and owned, or just documented?
- Practice reality — Do staff know what to do, and do they actually do it?
- Recording and reporting — Are concerns captured accurately, escalated properly and followed up?
- Leadership oversight — Do managers and boards know what is really happening?
- Resident and family voice — Are the people in care, and their families, genuinely heard?
- Patterns and themes — What do incidents, near-misses and concerns tell you over time?
What You Receive
- A clear, evidence-led written report with findings and observations
- Thematic analysis identifying patterns, risks and areas of concern
- Practical, proportionate recommendations for improvement
- A reflective summary for boards and governance reporting
- Follow-up support to review progress (where agreed)
Important: ORVIA is not a regulator, not an enforcement body, and not a replacement for statutory safeguarding services. Our safeguarding audits provide independent insight and reflective oversight — we do not carry out statutory investigations or replace the role of CQC, local authority safeguarding teams, or the police.

Frequently Asked Questions
How long does a safeguarding audit take?
This depends on the size and complexity of the service. A single-site audit typically takes 2–5 days of on-site review, with reporting delivered within an agreed timeframe. Multi-site audits are scoped individually.
Is a safeguarding audit the same as a CQC inspection?
No. CQC inspections are regulatory assessments carried out under statutory powers. A safeguarding audit by ORVIA is an independent, reflective review — it looks deeper at culture, practice and operational reality, but does not carry regulatory authority.
Can a safeguarding audit be used as evidence?
Our reports provide independent, evidence-led observations and findings. They can inform governance, commissioning decisions and improvement planning. They are not forensic investigations and should not be treated as legal evidence without appropriate advice.
Do you audit all types of care settings?
Yes. We work across residential care homes, supported living, learning disability and autism services, mental health settings, domiciliary care and NHS environments. See all settings we support.
What if we’ve already had a CQC inspection?
A safeguarding audit complements regulatory inspection. CQC provides a point-in-time assessment. We look at the ongoing culture and practice between inspections — helping you see what CQC may not have time to examine in depth.
How much does a safeguarding audit cost?
Costs depend on scope, setting size and complexity. Start a conversation with us to discuss your needs and we’ll provide a clear, transparent proposal.
