What Is an Independent Safeguarding Review?
An independent safeguarding review is a structured examination of safeguarding concerns, incidents or practice carried out by someone with no connection to the organisation under review. Independence matters because it removes the bias, blind spots and internal politics that can prevent organisations from seeing problems clearly.
ORVIA conducts independent safeguarding reviews that are reflective, not adversarial. We seek to understand what happened, identify systemic issues, and recommend meaningful changes — not to blame individuals or produce tick-box findings.
When Is an Independent Safeguarding Review Needed?
- Following a serious safeguarding incident or near-miss
- When internal review has not resolved concerns
- When families have lost confidence in a provider’s response
- As part of a Safeguarding Adults Review (SAR) or Domestic Homicide Review (DHR) process
- When a commissioner needs independent assurance about a provider
- When patterns of concern suggest systemic rather than individual failings
- When staff have raised whistleblowing concerns about safeguarding
How We Conduct Independent Reviews
Our approach follows the ORVIA framework — Observation, Reflection, Visibility, Insight, Accountability:
- Scoping — We agree the terms of reference, scope and boundaries with the commissioner
- Evidence gathering — Documentary review, observation, and structured conversations with staff, residents and families (as appropriate and agreed)
- Analysis — We examine findings through a reflective lens — looking for patterns, systemic issues and cultural factors
- Reporting — Clear, evidence-led report with findings, observations and recommendations
- Debrief — We present findings to the commissioner and discuss implications

Frequently Asked Questions
What’s the difference between an independent review and an investigation?
An investigation typically implies statutory or legal authority to compel evidence and make binding findings. ORVIA conducts reflective reviews — we examine evidence, observe practice and report findings independently, but we do not carry statutory investigation powers. Where a formal investigation is required, we can advise on appropriate routes.
Who can commission an independent safeguarding review?
Care providers, local authority commissioners, NHS ICBs, housing associations and families. The commissioning arrangement is agreed at the outset, including scope, access and reporting.
How long does an independent review take?
This depends on complexity. A focused single-incident review may take 2–4 weeks. More complex reviews involving multiple concerns, settings or timeframes may take longer. We agree realistic timescales at scoping stage.
Will you share findings with CQC or the local authority?
Our report belongs to the commissioner. We agree reporting and disclosure arrangements at the outset. If we identify immediate risk to safety during a review, we will discuss escalation with the commissioner.
