ORVIA 360°

The 360° Assessment

A complete, independent review of how your service really operates — from safeguarding and governance to culture and lived experience.

ORVIA 360° Assessment — independent safeguarding review

What We Examine

The 360° Assessment uses ORVIA’s six-layer model to examine every dimension of your service. We do not inspect from a desk. We observe, interview, cross-reference and test — because the truth of a service is in its corridors, not its policies.

🛡️

Safeguarding

Referral patterns and quality. Incident and complaint trends over 12+ months. Are concerns being raised — and are they being heard?

⚖️

Governance

Board minutes and decision-making patterns. Are leaders visible and accountable, or operating at distance from the people they serve?

👥

Workforce

Staffing levels, turnover, agency usage and supervision records. A service is only as strong as the people delivering it — and how well they are supported.

🏥

Culture

What does it feel like to be in this service? For staff, for residents, for families. Culture is the invisible architecture that either protects people or puts them at risk.

📊

Leadership

Leadership visibility and accessibility. Do managers walk the floors? Do staff feel safe to raise concerns? The answers tell us everything.

👨‍👩‍👧

Lived Experience

Family engagement and complaints handling. The voice of the people receiving care is the most important evidence in any review.

“Safeguarding starts and ends with the person. Not the policy. Not the procedure. The person.”

— The ORVIA Principle

“We cross-reference all findings against published safeguarding adult reviews, CQC thematic findings and sector-specific evidence — to identify patterns your own governance may be missing.”

— ORVIA Methodology

What You Receive

Every 360° Assessment produces a clear, evidence-led output that can be shared with regulators, commissioners, legal professionals and safeguarding boards.

📝 Full Report

Evidence-led findings with clear recommendations. Not a tick-box exercise — a genuine assessment of what is working, what is not, and what needs to change.

📋 Action Plan

Prioritised improvements with realistic timelines. We tell you what to fix first, what can wait, and what requires immediate attention.

🗣️ Review Session

Face-to-face or virtual debrief with your leadership team. We walk through every finding, answer questions, and agree next steps together.

📊 Executive Summary

Board-level summary for governance reporting. Suitable for contract monitoring evidence, commissioner assurance, and regulatory submissions.

How We Work

Every assessment is scoped to your service. We review documents before we arrive, observe on the ground, speak to staff and families, and cross-reference everything against national evidence.

1️⃣

Scoping

We discuss your service, understand the context, and agree the scope. You receive a clear, fixed quote before any work begins. Initial conversations are always free.

2️⃣

Assessment

On-site observation, staff interviews, document review, family engagement and OSINT where relevant. We look at what is happening — not what the paperwork says should be happening.

3️⃣

Report & Action

Full evidence report, prioritised action plan, and a face-to-face debrief. We do not just identify problems — we give you a roadmap to fix them.

FOR COMMISSIONERS

Evidence Your Board Can Rely On

ORVIA 360° reports are structured to meet commissioner assurance requirements. We work independently from the provider, so our findings are free from conflict of interest.

CQC Aligned

Findings aligned with CQC key lines of enquiry and compatible with local authority quality assurance frameworks.

Contract Monitoring

Suitable for contract monitoring evidence, safeguarding adults reviews and serious incident investigations.

Shareable

Reports can be shared with regulators, safeguarding boards and legal professionals. Executive summary included for board-level consumption.

Know the Truth of Your Service

Common Questions

How long does a 360° Assessment take?
It depends on the size and complexity of your service. A single care home may take 2–3 days on site plus reporting time. A larger organisation or multi-site review will take longer. We agree the scope and timeline upfront.
What does it cost?
Every 360° Assessment is scoped individually based on the size and complexity of your service. We provide a clear, fixed quote before any work begins. Initial conversations are always free. Contact us at hello@orviahealthcare.co.uk or 0114 399 8231.
Is ORVIA a regulator?
No. ORVIA is an independent safeguarding and operational oversight organisation. We are not a regulator, not a consultancy and not part of your governance structure. That independence is what makes our findings credible.
Can the report be shared with CQC or commissioners?
Yes. Reports are structured to meet commissioner assurance requirements and can be shared with regulators, safeguarding boards and legal professionals. We include an executive summary for board-level consumption.
Do you work with schools as well as care providers?
Yes. The 360° Assessment applies to any organisation with a duty of care — care providers, schools, supported living, mental health services, children’s homes, and more. The methodology adapts to the setting.
What if we are already under regulatory scrutiny?
That is exactly when an independent 360° Assessment is most valuable. We can provide the evidence base to support your improvement plan, give commissioners confidence in your response, and identify what your own governance may be missing. We work discreetly and professionally.
